Charges for Services

The Columbia/ Boone County Department of Public Health and Human Services provides many free services to the general public.In addition, we charge nominal fees for certain other services.  No one will be denied services for inability to pay, but all patients will be asked to pay according to the following schedule*:

Vaccines	Fee
Pediatric vaccinations provided by state and federal programs (school shots)	$15 per visit administration fee
Pneumovax - pneumonia vaccine for older adults and immune compromised	$40 Free for those over 65
HPV cervical cancer vaccine for women and girls age 9 thru 26	$140 per dose in 3 dose series
HPV cervical cancer vaccine for  girls and women age 9 thru 18, who have no insurance, or are on Medicaid  (VFC eligible)	$10.00 per dose
Adult Hepatitis A vaccine - 2 doses required	$25 per dose
Adult Hepatitis B vaccine - 3 doses required	$30 per dose
"TwinRIx" Hep A/Hep B Combination vaccine - 3 doses required	$45 per dose
Varicella vaccine - Chickenpox	$15
TD (Tetanus/Diptheria) vaccine- for adults	$15
TdaP (Tetanus, Diptheria and Pertussis for adults)	$15
Meningococcal vaccine (18 years of age or younger)	$15
Meningococcal Vaccine (all others)	$100
MMR (Measles, Mumps, and Rubella) vaccine	$15
Polio vaccine- for adults	$15
Seasonal vaccinations - flu shots from age 6 months and FluMist nasal vaccine for healthy people age 2 to 50 (Novel H1N1 vaccine is free. )	$25 - we cannot accept private insurance but will bill to Medicaid and Medicare

STD Testing and treatment for Boone County Residents	$10
STD Testing and treatment for persons from outside Boone County	$20
Herpes type1 and 2 blood tests	$25
Herpes Culture	$35
Pregnancy testing	$5 for teens $10 for adults
Lead testing (Children)	$15
Lead Testing (Adults)	$15

*Fees effective Oct 1, 2008 may vary with income, age, and insurance status.
