Birth or Death Certificates

How do I get a birth or death certificate?

Please visit our office or mail this completed application (English Form or Spanish Form) with payment to the health department at:

On what days and times can I apply for a birth/death certificate?

We are available Monday-Friday from 8:00 am - 5:00 pm. You can also mail your completed application with payment to the address listed above. Please note that we are closed on the following holidays: New Years Day, Martin Luther King, Jr. Day, Memorial Day, July 4th (Friday before if the 4th falls on a Saturday, Monday after if the 4th falls on a Sunday), Labor Day, Thanksgiving Day, and Christmas Day.

Who can request a birth or death certificate?

For privacy purposes, the following individuals are the only permitted persons that are able to request a birth or death certificate.

How much do birth certificates cost?

Birth certificates are $15 per certified copy. Payments can only be made using cash, check or money order.

How much do death certificates cost?

Death certificates are $13 for the first copy and $10 for each additional copy. Payments can only be made using cash, check or money order.
