How do I get a certified birth or death certificate?
There are three easy ways to get a Missouri birth or death certificate:
In-personat our office Monday - Friday from 8 am - 5 pm.
Applications are available at our office, or you may print and complete the application before coming in. English application or Spanish application
By mailing your completed application and payment to:

Online through our secure partnership with VitalChek (additional fees will apply) Columbia/Boone County Department of Public Health and Human Services does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network Inc. Please note that we are closed on the following holidays: New Years Day, Martin Luther King, Jr. Day, Memorial Day, July 4th (Friday before if the 4th falls on a Saturday, Monday after if the 4th falls on a Sunday), Labor Day, Thanksgiving Day, and Christmas Day (Friday before if the 25th falls on a Saturday, Monday after if the 25th falls on a Sunday).
Who can request a certified birth or death certificate?
For privacy purposes, the following individuals are the only permitted persons that are able to request a birth or death certificate.

Immediate family member
Legal guardian (Please bring your guardianship papers)

How much do certified birth certificates cost?
Birth certificates ordered in our office or through the mail are $15 per copy. You can pay with cash, check, money order, debit card or credit card. We accept
Visa, Mastercard and Discover. Birth certificates ordered online through VitalChek are $26.25 ($15 for the birth certificate and $11.25 for VitalChek's handling fee).
How much do certified death certificates cost?
Death certificates ordered in our office or through the mail are $13 for the first copy and $10 for each additional copy. You can pay with cash, check, money order, debit card or credit card. We accept
Visa, Mastercard and Discover. Death certificates ordered online through VitalChek are $24.25 ($13 for the death certificate and $11.25 for VitalChek's handling fee).
Please note:
The Health Department/Registrar’s Office has access to Missouri birth records only from 1920 to the present. Missouri death records are available from 1980 to the current year.
Not all records will be available for printing due to various reasons.
