For over 50 years the
City of Columbia has been committed to providing public health services through its
Health Department.
These services prevent epidemics and the spread of disease, protect against environmental hazards, promote healthy behaviors, prevent injuries and illness, assure the quality and accessibility of health services and monitor the health of the population.

How do I get a birth or death certificate?

Please visit us or mail us a completed application ( English Form or Spanish Form ) and payment to the Health Department.

On what days and times can I apply for a birth/death certificate?

We are available to assist you during the following times:

Monday	8:00 am – 5:00 pm
Tuesday	8:00 am – 6:00 pm
Wednesday	8:00 am – 5:00 pm
Thursday	8:00 am – 5:00 pm
Friday	8:00 am – 5:00 pm

Please note that we are closed on the following holidays: New Years Day, Martin Luther King Day, Memorial Day, July 4th, Labor Day, Thanksgiving, and Christmas Day

Who can request a birth or death certificate?

For privacy purposes, the following individuals are the only permitted persons that are able to request a birth or death certificate.

Registrant
Immediate family member
Legal guardian (Please bring your guardianship papers)
Legal representative

How much do birth certificates cost?

Birth certificates are $15 per certified copy. Payments can only be made using cash, check or money order.

How much do death certificates cost?

Birth certificates are $13 for the first copy and $10 for each additional copy. Payments can only be made using cash, check or money order.

Please note:

The Health Department/Registrar’s Office has access to Missouri birth records only from 1920 to the present. Missouri death records are available from 1980 to the current year.
Not all records will be available for printing due to various reasons
